Neighbors Light is designed to work through partnerships with shelters, hospitals, case managers, and service providers. The program is referral-based and intended to support individuals who are ready to move beyond immediate crisis services but need structured support to do so successfully.
By working alongside existing community organizations, Neighbors Light complements the services you provide and helps create a coordinated pathway from crisis toward long-term stability. We are not here to replace existing services, but to fill a critical gap in the continuum of care.
Identify individuals who are ready to move beyond immediate crisis services and benefit from structured stabilization support.
Connect patients who are medically stable but need structured recovery housing and support during the transition home.
Refer clients who are ready for the next step and need time-limited, goal-oriented support to prepare for independence.
Work with workforce programs, nonprofits, and community agencies to create coordinated pathways for individuals you serve.
Referrals will be accepted through partner organizations once Neighbors Light is operational. This ensures that all participants are connected to a trusted partner who understands their needs and can support their transition.
Walk-in placement will not be available. All participants will be referred by trusted partner organizations to ensure we can provide the right level of care and support.
Organizations interested in partnering with Neighbors Light will work with our team to establish referral protocols and communication channels.
Partners will submit referrals through a streamlined process, providing relevant information about the individual and their readiness for stabilization support.
Accepted individuals will complete intake with our team, establish goals, and begin their stabilization journey with Neighbors Light.
